**Job Overview** Fairview is seeking a **Family Medicine Physician** to join our primary care practice at M Health Fairview Clinic - Cottage Grove. Our clinic offers a welcoming, small-town feel while also providing patients with access to outstanding resources and system-wide support. **Practice Details:** + **9 provider primary care** practice which includes 5 Primary Care Physicians and 4 Advanced Practice Providers. + Patient encounters average **15-18** **patients per day.** + **Opening: 1.0** **FTE - flexible schedule.** + **Call Frequency:** **1:3** **low intensity - phone consultation only with first tier nurse triage** + **Fully integrated** Electronic Medical Record (EPIC). + Examines, diagnoses and treats patients of all ages, for acute injuries, infections, illnesses and counsels' patients and families about acute and chronic conditions or concerns. **Responsibilities** + Examines patients, formulates differential diagnostic plans, defines and orders required diagnostic testing. Provides preventative, chronic disease, and acute care depending on the need of the clinic site. Ensures proper injury care and disease prevention, diagnosis, treatment, and recovery. + Prescribes and administers medications, performs routine vaccinations, and provides advice regarding personal health and hygiene. + Interprets examination findings and test results and implements treatment plans. Refers patient to medical specialist or another practitioner when necessary. + Completes medical record and electronic order management (via in basket); and completes cross coverage depending on site needs. Follows organization policies and procedures and protocols. + Actively participates in multidisciplinary team approach to case management + Participates in utilization and quality tools and activities that promote continuous improvement in delivery of patient care, patient outcomes or workflows. + Maintains knowledge of, and complies with, all relevant laws, regulations, policies, procedures, and standards. + Actively participates in creating and implementing improvements. **Required Qualifications** + Graduate of an accredited ACGME/AOA Family Medicine residency training program + BC/BE with the ABMS or AOA + Licensed or ability to obtain licensure through the Minnesota Board of Medical Practice + Ability to obtain and/or maintain DEA certification for the State of Minnesota + Meet threshold criteria/qualification for Credentialing and Privileges + Board Certified in Family Medicine, or dual Board Certification in Internal Medicine/Pediatrics Minnesota license to practice as an MD or DO M Health Fairview Clinic - Cottage Grove ( is located in the picturesque suburb of Cottage Grove just 10 miles south of St. Paul near the Mississippi River, St. Croix River, and Afton Alps State park (~10 minutes from Woodbury, MN). + + + + **Benefit Overview** Fairview offers a generous benefit package, including but not limited to medical, dental, vision plans, life insurance, short-term and long-term disability insurance, PTO and Sick and Safe Time, CME, tuition reimbursement, retirement, and more! The actual rate of pay offered within this range may depend on several factors, such as FTE, skills, knowledge, relevant education, experience, and market conditions.
