Job Summary:
As a Clinical Assessor at Acentra Health, you will make a meaningful impact in your community by conducting comprehensive, needs-based eligibility assessments for individuals seeking Medicaid-funded personal care services. Your work will determine whether services are provided in the individual’s home or in an adult care or supervised living setting across Portsmouth, New Hampshire, and surrounding areas.
Additionally, you’ll assess Level of Care (LOC) to help individuals remain in or return to home and community-based settings. Your assessments will inform care planning, support appropriate placements, and ensure smooth transitions to long-term care. By identifying individual needs and eligibility, you will contribute directly to improving health outcomes, enhancing quality of life, and promoting overall well-being.
This PRN hybrid role offers flexibility and purpose, combining remote work from your home office with local travel as needed. Travel expenses including mileage and parking are covered and reimbursed. In addition, Acentra provides all necessary technology equipment, including a laptop for use in the field.
Responsibilities:
Conduct comprehensive assessments to determine if beneficiaries meet eligibility criteria for Personal Care Services (PCS) using state-approved standardized assessment tools.
Evaluate and document PCS needs based on medical conditions, functional limitations, and physical or cognitive impairments to ensure services are provided appropriately and on a needs-based basis.
Uphold the highest standards of privacy, respect, and dignity throughout the assessment process for all beneficiaries.
Complete all assigned assessments—including new, expedited, annual, change of status, mediation/appeals, reconsideration, and derivative reviews—within required timeframes.
Engage with family members and informal caregivers present during assessments to gather relevant information and support the beneficiary’s care planning.
Assist beneficiaries in understanding and selecting appropriate PCS providers as needed.
Conduct service plan reviews and updates as required to ensure alignment with the beneficiary’s current care needs.
Accurately submit completed assessments through the state-approved interface in a timely and compliant manner.
Participate in mediation and appeals processes on behalf of the beneficiary when needed.
Respond to state agency inquiries and provide clarification or additional information regarding completed assessments.
Actively contribute to team meetings, peer reviews, internal audits, and case consultations as assigned.
Complete eligibility assessments for applicants entering the 1915(c) Home and Community-Based Services (HCBS) program, and conduct annual or status-change assessments for current participants, using approved assessment tools.
Comply with all corporate policies and procedures, including strict adherence to HIPAA Privacy and Security Rules to protect beneficiary data and confidentiality.
Must hold an active, unrestricted Registered Nurse (RN) or Licensed Clinical Social Worker (LCSW) license to practice in New Hampshire.
Graduation from an accredited college or university with a degree in a relevant healthcare or human services field.
Demonstrated expertise in clinical assessment and critical thinking, ensuring accurate evaluations, determinations, and care recommendations.
Medical record review and abstracting, with a strong understanding of medical terminology, clinical documentation, record organization, and disease processes.
Strong verbal, written, and electronic communication skills, with the ability to clearly convey complex information and collaborate effectively with individuals and teams across various platforms.
Preferred Qualifications and Experience
Familiarity with public behavioral health, medical programs, and Medicaid services.

Compensation
The compensation for this role ranges from $34.00 to $38.00 per hour.
